Not that I imagine you're very interested in my opinion since I fall into an age category that is clearly an 'everybody else' - 4 divisions under 30, then 'anyone else'. Cheers.
I wish you luck with your project because I'm sure that you are trying to gather useful data, but can you tighten this up a bit? Why does TV have a 'rarely' option instead of 'occasionally'? And what does 'all the time' mean here?
And is the question "What is your favourite -animation -short film" a multiple choice? I ask, because all the others are; do you want people to choose between them?
I'm not trying to be nasty, but this needs a bit more thought, doesn't it?
Additional thought (if you're still with me). At the risk of sounding patronizing, have you thought about what you want to get from your research? If you want to write/make a good film, then shouldn't some of your questionnaire ask about why people like the shorts they like? You could say something along the lines of "I never watch shorts because they are usually ...", "I only like shorts animations that are ..." "I hate short films that ... " "My favourite ever short was ... because it was so ...". Or actual questions, as opposed to finishing sentences.
With your questionnaire as is, the most you will get is an idea of how frequently people watch any film that is short and how often they watch TV. Will that help you enough?
